optimize digital health technology in partnership with vendors and system collaborators

Integrated Neighbourhood Home Care Program

Closing the Gap is a proud partner of the Integrated Neighbourhood Home Care Program, an initiative in the Thorncliffe Park and Taylor-Massey neighbourhoods in East Toronto that aims to improve home care services for adults.

Together, we are making home care even better and more connected for clients and healthcare providers in this vibrant community. The Integrated Neighbourhood Home Care Program is a proud initiative of East Toronto Health Partners Ontario Health Team (ETHP OHT).
